Provider Portal User Agreement

Welcome to the Provider Portal. Before you continue, please read carefully:

  1. Use the portal only for work
    • Check member eligibility.
    • Send claims.
    • Request pre-authorizations.
    • View patient info only when needed for care or claims.
  2. Protect patient information
    • Keep it private and secure.
    • Don’t copy, download, or share outside what’s needed for treatment or claims.
    • Follow all privacy and health data laws.
  3. Keep your login safe
    • Your username and password are for you only.
    • If staff leave your organization, their access must be removed right away.
    • Anything done under your login is your responsibility.
  4. No misuse
    • Don’t use the portal for marketing, research, or any personal purpose.
    • Don’t try to break security, copy large amounts of data, or tamper with the system.
  5. We monitor activity
    • Insurer and Life Quest (TPA) may log and audit portal use to protect patient data.
  6. Terms may change
    • If the terms are updated, you’ll be asked to review and accept them again.
    • Continuing to use the portal means you agree to the updated terms.
